Prior to accessing your electronic medical records, you must register by signing and returning to us, the Electronic Medical Records (EMR) Terms And Conditions. We can mail you the form or you can download it below.

Here is a list of recommendations by the American Academy of Family Physicians (AAFP) for a number of periodic health interventions.

These recommendations are provided only as an assistance for physicians making clinical decisions regarding the care of their patients.
